Health Insurance Plans

Our urologists have contracts with the following health insurance plans. If you do not see your insurance company on this list, please do not assume that we cannot provide your health care. A good way to find out if our doctors are contracted with your insurance is to call the benefits/customer service phone number listed on your insurance card. If you discover that our doctors are not contracted, we are happy to discuss your insurance coverage with you and work out the best possible financial arrangement.

	¹Dr. Levine is not contracted with Blue Choice POS, Unicare POS or Unicare HMO. He is considered an out-of-network physician with all of these plans.

In order to provide the best possible service to you, our insurance plan contracting is done through the Rush PHO (Physican Hospital Organization). This relationship insures that our physicians are contracted with the same insurance plans as the hospital. Before your surgery, please speak with our billing department if you have questions or are concerned that insurance will not cover your procedure.
