 Scheduling an Appointment
Office Hours
Our office is open Monday through Friday, 8:30 am to 4:30 pm. Please call 312.563.5000 during office hours to schedule an appointment. Should an emergency arise outside of office hours, we do have a 24-hour answering service available at this same number.
Your Appointment
We are aware that your time is valuable to you. We strive to run our schedules on time although occasionally an emergency will delay us. We ask that you please arrive 15 minutes before your appointed time to allow time for administrative tasks. If, for any reason, you need to cancel or reschedule your appointment, please notify us as soon as possible so that we may accommodate other patients waiting to be seen.

If this is your first appointment, we find it is most helpful to prepare for your visit ahead of time as this will ultimately save you time in the long run.

Please assist us by bringing the following to your appointment:
  • Registration information: You can access and print these forms on our Web site. Please be sure to complete each form in its entirety and present all forms to the front desk when you check in. All patients will be asked to complete the Registration Form, Signature Form, Payment Policy, and History Form. Questionnaires are also available for specific medical conditions. Please consult our forms page for more information.
  • If for any reason you have not received or completed your registration packet prior to your appointment, you should arrive at our office 20 minutes before your scheduled appointment time to allow time for this to be completed.

  • Contact information for the physician who is sending you to our office: In order to provide comprehensive care, we will report back to the physician who sent you to our office. To assist us, please provide this physician's name, address, and telephone number.

  • Current insurance cards and a photo ID: You must bring your insurance cards and a photo ID with you. If you do not have your insurance cards present, you will be expected to pay in full for that day's services. We accept cash, checks, Visa, Mastercard and Discover.
  • Be prepared to pay your copay and/or deductibles when you are seen. If you do not have insurance, you should be prepared to pay for your visit.

  • Referral form (if required): Depending on your insurance carrier and specific coverage, you may require a referral to see a specialist. Please check with the customer service department of your insurance company or your PCP if you have questions regarding referrals. Failure to provide a valid referral prior to service will result in your being personally responsible for payment of any charges incurred.

NOTE: If this is your first appointment and you are seeing Dr. Levine for a Peyronie's disease consultation, you will need to first schedule a Penile Duplex Ultrasound in our office before seeing the doctor. This test cannot be done at another facility. We try to accommodate both the ultrasound and consultation on the same day; however, this may not always be possible. Please be sure to complete the Peyronie's disease questionnaire in addition to the registration paperwork. For more information, please see the forms page.
